What information will you need to report a death to Prudential?
You will need to have the following information to hand when telling Prudential of a death:
- Full name, date of birth and home address of the deceased
- Date of death
- Your contact details, as the notifier
There is some extra information that will help speed up the process, but they can still accept the form without it:
- Product and Policy information (which can be found on the policy documentation)
- Contact details of person(s) dealing with the estate such as executor(s) or next of kin
- Death certificate details
